For example, if a nurse failed to wash their hands after treating a patient they knew was infected with, say, C. diff, then a patient who was later infected may have a case. But if the nurse followed all standard procedures and a patient still contracted C. diff, then there would be no case. The infection must have been preventable for the patient to have a case.
